| Options Workgroup: Micro Strategies To Develop LTC
Integration Goal: To accomplish the benefits of integrating long term care into the mainstream of health and social service delivery in San Diego County, multiple complementary strategies will be pursued. The idea, as proposed by Dr. Mark Meiners, is to develop key components of care systems both within fully integrated care plans as well as through stand-alone options. All the strategies described below will assist in the coordination and access of all populations with chronic care needs to the full continuum of quality health and social services. |

| Strategy Group I: Network of Care
(single point of information and access) Goals: 1. Design pilot to survey/assess consumer and provider satisfaction and potential improvements, additions to the Network of Care Program. 2. Estimate resources needed to complete survey and make program changes. 3. Design outcome measures to assess effectiveness of pilot. |

| Strategy Group II: Physician Strategy Goals: 1. Design pilot to identify physician groups, who work with elderly and disabled, who would be interested in coordinating all health and social services for a designated number of individuals with the help of care management resources. 2. Examine resources needed for physician incentives, physician outreach and education, and care management support and identify potential funding. 3. Design outcome measures to assess effectiveness of pilot. |

| Strategy Group III: Health Plans/Pilots Goals: 1.A. Design survey to assess characteristics of current ABD population (approximately 10,000) voluntarily enrolled in Healthy San Diego (HSD). 1.B. Identify voluntary pilots to test integration models in San Diego. 2.A. Estimate resources to survey and compile data from ABDs in HSD. 2.B. Examine resources needed to assist pilots in implementation, and identify potential funding. 3.A. Design output desired from survey information. 3.B. Design cost and quality effectiveness indicators for pilots. |
