On January 9, 2008, more than 50 stakeholders participated in discussion regarding the TEAM SAN DIEGO draft curriculum after the panel presentation on challenges to successful transitions across settings and providers. TEAM SAN DIEGO is our community education initiative to change the local culture where a person’s providers, caregivers, and the individual take joint responsibility as a “team” for improved chronic care outcomes. Panelists included our own distinguished stakeholders, whose qualifications can be found on the back of the Agenda.

November 14, 2007 Planning Committee Meeting Material:
Thanks to all who joined us on for the Planning Committee Meeting on November 14, 2007 for the physician panel on chronic care management in the office, clinic, or home setting. Doctors White and Romero provided us all with a challenge to see if the draft TEAM SAN DIEGO curriculum addressed the problems identified by our physicians. available now!! The federal grant from AoA and CMS was successfully completed in June of 2007 in Del Norte and San Diego County and the report describes the details!

The new “virtual” Long Term Care (LTC) Options Counselor is now available on the Aging and Disability Resource Connection Web site at www.sandiego.networkofcare.org/aging. Once you log onto the home page, scroll down the page and click on the “LTC Options Counselor” button located on the bottom left. This basic planning guide is designed to help individuals think about and navigate their way through an array of choices and information on planning for their health, quality of life and future home and community-based needs.We want your feedback! Over 50 stakeholders participated in the February 14, 2007 Planning Committee meeting. Thank you to everyone who attended! In the morning, Heather-Johnson Lemarche, MSW, MPH, presented via teleconference on the successes and challenges in supporting the integration of philosophies between the aging and disability communities within the state of Massachusetts and Vermont's Aging and Disability Resource Center (ADRC) initiatives. Heather’s expertise provided stakeholders and staff with valuable insights that will help strengthen San Diego County's ADRC partnership between Aging & Independence Services (AIS) and Access to Independence, the local Independent Living Center. Click on the links below for more information about the meeting:


The December 13 Planning Committee meeting featured presentations on activities related to San Diego's Aging & Disability Resource Connection Strategy (aka Network of Care) and partnership with the Access Center of San Diego under new supplemental grant resources. California was awarded a supplemental grant in the amount of $83,000 to strengthen San Diego’s ADRC partnership between AIS and Access to Independence, the local Independent Living Center and a leader in disability services! The Network of Care (www.sandiego.networkofcare.org) is a newly enhanced resource for San Diego’s “Aging and Disability Resource Connection” (ADRC). The August 9 Planning Committee meeting provided an overview of Healthy San Diego, the local Geographic Medi-Cal Managed Care model. This informative presentation provided important information for future planning on how Healthy San Diego Plus will operationalize under this model. Click on the links below for more detail:

 

In June, approximately 60 stakeholders heard updates on the third-year grant goals for the Aging & Disability Resource Center (ADRC) initiative. Lora Connolly, Director, California Department of Aging, and Ben Harville, Department of Rehabilitation, participated. Dr. Mark Meiners moderated the panel, which included Afshin Khosravi, Trilogy Integrated Resources, and Louis Frick, San Diego Access Center. The focus was on highlighting enhancements to the Network of Care website under the ADRC initiative, including a new Google-like search, and setting additional goals for improving the ADCR for use by the younger disabled community. On May 9, 2006, the Board of Supervisors approved moving forward with the Physician Strategy Implementation Plan and development of the Healthy San Diego Plus (HSD+) voluntary pilot for acute and long term care integration, based on the Governor’s proposal. Staff will return to the Board for acceptance of grant revenue when additional grant funding is identified. Click here to download a copy of the Board letter.

The May 10 Planning Committee meeting focused on the Physician Strategy, with presentations and group discussion around two fee-for-service Medicare demonstration projects. Both demos represent examples of improved care management strategies for persons with chronic conditions. Click on the links below for more information:

The March 8 and April 12 Planning Committee meetings included an overview of LTCIP’s three strategies, including discussion of the Governor’s pilot proposal for acute and long term care integration. The majority of the group supported moving forward with the Governor’s proposal if the LTCIP Personal Care Services Workgroup pilot recommendation was determined not to be feasible. The Governor’s proposal was published on February 17, 2006 and legislation has been sponsored by Richman (AB 2979). The bill was last amended on May 15, 2006.

At the February 8 Planning Committee meeting, stakeholders were presented with an overview of the Governor’s Budget Proposal as well as recommendations forwarded by the Personal Care Services (PCS) Workgroup, which included a pilot proposal to integrate In-Home Supportive Services (IHSS) on a limited basis. A majority of stakeholders supported moving forward with pilot developed by the PCS Workgroup. Some stakeholders abstained from the vote to support the pilot until the legislative language for the Governor’s proposal was published. The January 11 Planning Committee meeting focused on the development of web support for acute and long term care integration on a broad basis. Stakeholder consensus was reached at the November 9, 2005 Planning Committee meeting on the revised draft recommendations for Care Management, Provider Network Adequacy & Education, Member Services/Education and Community & Cultural Responsiveness minimum requirements for Healthy San Diego Plus (HSD+). These recommendations expand upon current Healthy San Diego/Geographic Managed Care (GMC) requirements and will be used to guide in the development of Request for Statement of Qualifications (RFSQ) language and subsequent contract terms for Acute and Long Term Care Integration (ALTCI) plans. The RFSQ is the process through which ALTCI plans will qualify locally to be approved to contract with the State to participate in HSD+. Medicare/Medicaid Integration Program (http://www.gmu.edu/departments/chpre/research/MMIP/index.html) - Center for Health Policy, Research and Ethics, George Mason University. MMIP has funded 14 states in various stages of planning, development, and implementation of programs seeking to integrate Medicaid's long-term care services with Medicare's acute care services through managed care.

California Department of Health Services Office of Long Term Care awards Implementation Grant to San Diego! After a period of negotiation with the State, the local LTCIP was awarded $423,333 to continue efforts toward the implementation of HSD+ by January 2007. San Diego’s goal is to complete intensive work, as set for in the Scope of Work, resulting in implementation recommendations with consultants during the 6-month Grant period, ending June 30, 2005. These recommendations will be presented to the stakeholders for input and consensus development between July and December 2005. Then these recommendations, with community-wide support, will be presented to the Board of Supervisors for further action early in 2006. Click on the links below for more information:

Broad and active participation of key stakeholders, providers, consumers and consumer advocates involved with the care of frail elderly and disabled in San Diego is pivotal to the success of the LTCIP planning process. To date, local stakeholders have devoted more than 22,000 hours of time and in-kind support to the LTCIP. All LTCIP planning meetings and workgroups are open to the public and we welcome new members. All new members should call LTCIP staff to confirm the date and time of the meeting posted.
