45-Day Notice of Hearing WaiverAllows you to waive the 45-day noticing period for an upcoming hearing.
Agent/Attorney Removal RequestTo remove an agent or attorney from your Application for Changed Assessment.
Agent's Authorization To Represent ApplicantAllows an agent to represent you on behalf of your Application for Changed Assessment.
Agreement to Extend Time FormTo be used when nearing the two-year deadline for action on your Application for Changed Assessment.
Contact Information Change Request for Appeals ApplicationUpdates the contact information on your application.
Postponements and Continuances (Resets)Request for a postponement of your scheduled hearing.
Re-Open RequestRequest to have your case re-opened.  Please view the form for more information.
Request to Withdraw Assessment AppealsUsed to withdraw your Application for Changed Assessment.  No further action will be taken on the withdrawn application.
