Purpose: The Child Health and Disability Prevention (CHDP) Program prevents and identifies health problems through periodic well-child health exams that help eligible children grow up healthy; and through links to ongoing health care coverage.
Facilities where this Service is offered:
Services
No-cost CHDP Program exams are available at certain ages to prevent, find and treat health problems early in your child's development.
- Review of health history
- Physical exam
- Height, weight and blood pressure measurements
- Vision and hearing tests
- Tests for anemia, blood lead poisoning and Tuberculosis
- Immunizations
- Dental Screening
New11-21 year olds, It's ur turn 2b healthy! Take charge of ur health!
Eligibility
Children and youth must live in California and meet one of the following requirements:
- Under 21 years of age and on Medi-Cal
- From a family of limited income
- Between the ages of 3 and 5 years and attend a Head Start or State Preschool
- In Foster Care
